Social Services 
A licensed social worker staffs the Social Services department. The function of the Social Services department is to assist patients and families in their discharge needs. Services provided include assistance in arranging transfers to other facilities such as Long-term Care, Substance Abuse, Rehab, and Mental Health facilities. The Social Services department also provides assistance in arranging for home care needs such as:
Home health
Medical equipment
Hospice care
Homemaker services
Home delivered meals
Financial assistance
The Social Services department works with the nursing staff and our care coordinators or Case Management staff in determining eligibility for admission to our Swing Bed program.
Our Social Worker provides comfort and assistance for families who experience the death of a loved one, by contacting clergy and other family members. Our Grief Support program is another service that is offered by the Social Services Department, which provides families with additional support following the loss of a loved one.
