Physical Therapy 
 A patient may be referred to physical therapy by a physician’s order. When the patient arrives at the Rehab and Wellness Center, the therapist conducts a thorough assessment of his/her condition. A report of the evaluation is then provided to the referring physician. Whenever possible, the patient is scheduled for services within 24 hours of receipt of orders. An individualized treatment program is designed for each patient and communicated to the physician. Each treatment plan closely follows the physician’s instructions when provided. Patient and family education is provided as appropriate. Treatment is provided for all ages.
