Health Information Management 
The Health Information Management Department (Medical Records) helps to produce, maintain, and protect confidential health information for each patient. We strive to produce quality information, code each record accurately, and release information following strict guidelines. We also strive to work with physicians, hospital staff, patients, and other customers in an effort to benefit the healthcare consumer and promote the values of Ohio County Hospital.
Confidentiality and security of health information is taken very seriously by the HIM Department. Please bring picture identification with you to obtain a copy of your medical records. You must sign a release of information to obtain or release your records. No one else can sign a release for your records except you, including your spouse. In a divorce case, proof of custody must be presented to obtain records on minor children. Also, legal documentation should be presented to obtain records on a deceased patient. The hospital does not have birth certificates. These must be obtained from the Office of Vital Statistics in Frankfort, Kentucky, (502) 564-4212 or http://publichealth.state.ky.us/vital.htm.
