Health Information Management The Health Information Management Department (Medical Records) helps produce, maintain and protect confidential health information for each patient. The HIM Department is committed to quality health information management. The HIM Department takes confidentiality and security of health information very seriously. Authorization must be obtained from the patient or individual allowed to act as a personal representative for the patient for the release of protected health information, unless it is otherwise allowed or required by law. |  |
Please bring picture identification with you to obtain a copy of your medical records. In a divorce case, proof of custody must be presented to obtain records on minor children. Legal documentation should be presented to obtain records on a deceased patient. The hospital does not have birth certificates. These must be obtained from the Office of Vital Statistics in Frankfort, Kentucky, (502) 564-4212 or http://chfs.ky.gov/dph/vital/birthcert.htm.
